Plan revisions:how crews end up building the wrong set

Every job site loses paperwork. Only one document failure reliably costs real money: a subcontractor building carefully, correctly, and entirely from a sheet that stopped being current three weeks ago.

A wet permit is annoying. A missing inspection card is a phone call. A superseded plan sheet is demolition, material ordered twice, a trade that leaves for another job, and an argument about who pays for all three.

The reason this failure is worse than the others is that it does not look like a failure while it is happening. Nobody skips a step. The framer reads the sheet, measures twice, and builds exactly what is drawn. The work passes his own inspection of it. It only becomes wrong later, when somebody holds up a different sheet with the same number on it and a different letter in the corner.

What follows is how revisions get issued, what the cloud and triangle on a redlined sheet actually mean, why the superseded set is the one that does the damage, how the rework plays out on a real schedule, and the working habits that keep a site on one set. How a revision actually gets issued

Section 01

A revision is not one kind of event. It is at least five, arriving from different directions, on different schedules, usually with different people copied on the email.

R1Plan-check corrections
The building department reviews the submitted set and returns comments. Sheets get redrawn to answer them. Many jurisdictions then stamp a corrected set, and that stamped set — not the one you bid — becomes the approved one.
R2RFI answers
A detail does not resolve in the field, somebody writes it up, and the architect or engineer answers. The answer is often a sketch first and a revised sheet later, which means two documents describe the same work for a while.
R3Field conditions
The wall is not where the survey said, the rock is higher than the soils report suggested, the existing structure is not what the as-builts showed. Somebody redraws around reality.
R4Owner changes
A moved island, a bigger opening, a different truss layout. These arrive late, feel small on paper, and frequently touch structural, mechanical, and electrical sheets at once.
R5Deferred submittals
Trusses, fire sprinkler, sometimes solar. The design is completed after permit issuance and commonly needs its own review before that work proceeds. The set you started with never had those sheets in it.

Whatever triggers it, the mechanics on the sheet itself are fairly consistent across offices. The drafter draws a cloud — an irregular, scalloped outline — around the specific area that changed, so a reader does not have to compare the whole sheet line by line to find what moved. Beside the cloud sits a small triangle (sometimes a hexagon, sometimes just a circled number, conventions vary by firm and discipline) containing a number or letter. That mark is a reference, not a label: it points to a row in the revision block in the title block, which lists the same number or letter, a date, and a short description of what changed.

Revision cloud
The scalloped outline. It marks the boundary of what actually changed on this issue of the sheet — everything inside it is new or different; everything outside it carried over unchanged.
Delta / revision triangle
The numbered marker next to the cloud. It ties that specific change back to a dated line in the revision block, so anyone reading the sheet can find out when the change happened and why.

Exactly how a given office draws this — triangle versus hexagon, letters versus numbers, one cloud per change versus one cloud per sheet — is a drafting convention, not a standard everyone follows the same way. If a sheet you are reading does not use clouds and triangles at all, ask whoever issued it how they mark changes before you assume a sheet with no clouds has had no changes.

The property that causes most of the trouble on site follows directly from this: revisions are issued by sheet, and often by the specific clouded area on that sheet. A revision rarely reissues the whole set. It reissues the sheet — or even just the one detail — that moved.

So “we are on Revision C” is not really a fact about the job. It is a fact about specific sheets, and sometimes about a specific cloud on one of them. The set on the plan table can be perfectly current on eleven sheets and three weeks stale on the one the framer is standing in front of, and nothing about the stack of paper will look wrong.

What happens next depends on where you build. Many building departments require revised drawings to be resubmitted and re-approved before the affected work proceeds, and commonly expect the approved revised sheets to be available on site alongside the permit. Some treat minor field changes differently from structural ones. None of that is uniform — confirm the process with your own building department rather than assuming the last jurisdiction you worked in transfers to this one.

Why the superseded set is the dangerous one

Section 02

Here is the part that is genuinely counterintuitive: issuing a revision does not remove the previous one. Every copy that was ever made is still out there, still legible, still opening perfectly well when tapped. Distribution creates copies, and copies do not expire. That is the entire reason the superseded set is more dangerous than a missing document — a missing document announces itself. A superseded one looks exactly as trustworthy as the current one, right up until it isn’t.

Printed sets
Somebody paid for those prints, so nobody throws them away. A retired set in a truck cab looks exactly like a current one from ten feet away — and from ten feet away is how sets get picked up.
Forwarded email
An attachment is a snapshot of the day it was sent. Forward it in October and it still opens; nothing about it announces that it stopped being true in July.
Photos of a sheet
A framer photographs the header detail so he does not have to walk back to the plan table. That photo is now, functionally, the plan. It lives in his camera roll and it never expires.
Shared folders
Plans_FINAL, Plans_FINAL_v2, Plans_REVISED_use_this_one. When the folder cannot answer which one is current, people open the one that opens fastest.
The bid set
Subs estimate from the set they were given at bid, and that is the file already saved on their laptop. Unless something replaces it, that is the file they build from.
The permit box
A weatherproof box on a post is a distribution channel with no expiry date. Whatever went in during framing is still in there at drywall, and it is nobody's specific job to clean it out.

One ordinary sequence

In March, a framer needs the header schedule on Sheet S-2 and does not want to walk back to the plan table for the tenth time that day, so he photographs the sheet. It is Rev A. In May, an RFI about a beam pocket bumps two header sizes on that same sheet to Rev C — the engineer reissues S-2, the superintendent gets the email, and the printed set on the plan table gets swapped for the new one. Nobody thinks about a photo sitting in a phone none of them can see.

In July, the same framer is back on a different opening along that wall line. He does not walk to the plan table — he never does, that is the whole point of having taken the photo — he opens his camera roll, because that is one tap and the plan table is a walk across an active site. He builds the Rev A header. Correctly, carefully, exactly as drawn. It is simply the wrong drawing, and nothing about the finished framing tells anyone that until an inspector, or the engineer, holds the current sheet next to it.

Nobody in that sequence did anything careless. The framer measured twice. The engineer answered the RFI promptly. The superintendent swapped the printed set the same week it arrived. The failure was that a photo taken in March had no way of knowing it had stopped being true, and no one told it.

Add the ordinary churn of a job — a sub who joins in month four and gets handed whatever the previous guy had, a superintendent covering two sites, a foreman who was out the week the revision landed — and the surprising thing is not that stale sets circulate. It is that the correct set ever wins.

It wins when it is the easiest one to reach. That is the whole mechanism, and everything in the habits section below is a variation on it.

How the rework actually happens

Section 03

The bill arrives in layers, and the direct labor to fix the mistake is usually the smallest one.

Demolition of good work. Somebody has to undo work that was built properly. That is labor paid twice for a net result of zero, and it is the most demoralising work on any job.

Material, twice. Some of it is salvageable. Cut lumber, mixed concrete, and anything already fastened generally is not. Long-lead items are the ones that hurt, because the second order arrives on the supplier’s schedule, not yours.

Schedule. The trade behind the mistake does not sit and wait — they leave for another job, and they come back when their calendar allows, not when yours does. This is why a two-day fix routinely turns into a two-week hole, and why the cost of the delay often exceeds the cost of the rework itself.

Inspections and cover. If the work was built to a superseded detail and then covered, opening it back up is its own line item. Re-inspection fees are common, and inspector availability is its own queue. Check what your jurisdiction charges and how it schedules re-inspections before you need to know.

The argument. Then comes the part nobody schedules for: whether this is a change order or a backcharge. That conversation turns entirely on evidence — which revision was current, when it was issued, and who was told. When nobody can show what was on site the day the work was done, the general contractor tends to absorb it, because the general contractor is the one who cannot prove otherwise.

And the layer with no invoice: a sub who ate a day of demo he did not cause prices your next job differently, or stops picking up the phone. Reputation among trades is a real asset and it is spent in exactly these moments.

General notes

Be suspicious of anyone — this site included — who quotes you an average cost of rework. It depends on the trade, the phase, what got covered, and who is holding the contract. The useful number is your own: next time it happens, write down the hours, the material, and the days lost. Two of those, on paper, will change how you distribute plans faster than any argument will.

Revision discipline that actually holds

None of this requires software. It requires that the true version be the most convenient version, and that the false ones get pulled out of circulation the same day.

    One address for the current set

    Not one file — one address. A link, a folder, a scannable code: something you can send in a text and still be right about six weeks later. The instant you send a copy of the set instead of a pointer to it, you have created a version that will outlive its accuracy.

  2. 02

    Retire the old set the hour the new one lands

    Retire, not destroy — you may well need the superseded set to prove what was current when. But it comes out of circulation immediately: off the plan table, out of the box, out of the shared folder people actually browse. A revision that is issued while the old set is still reachable has not really been issued.

  3. 03

    Make the current set the easiest thing to reach

    Convenience beats policy on every job site ever run. If the current sheet takes four taps and a login, and the photo in the camera roll takes one, the photo wins — and it will keep winning no matter how the pre-construction meeting went.

  4. 04

    Put the revision where a phone can read it

    The revision block in the title block is correct and unreadable at arm’s length on a cracked screen. Carry the sheet number, revision, and date in the file name too, so the identity of the document survives being forwarded, downloaded, and reopened.

  5. 05

    Track revisions by sheet, not by set

    Sets are rarely reissued whole. Revision C usually means four sheets changed and the rest did not, so “we are on C” is not a fact about the job — it is a fact about specific sheets. Ask which sheets moved, and confirm the trade in front of you is holding those.

  6. 06

    Name who gets told, per revision

    Keep one line per revision: sheet, revision, date, what changed, who was notified. It takes a minute, it makes the pattern of a job visible, and it is the document you will want if the question later becomes who should have known.

  7. 07

    Ask the revision question out loud before work starts

    At the pre-pour, pre-cover, and pre-drywall huddle, make the trade say the sheet number, revision, and date they are working from. Not “you got the latest?” — everybody answers yes to that. The letter and the date, said out loud, catch the mismatch while it is still free to fix.

  8. 08

    Treat a photographed sheet as a defect report

    When a sub photographs a plan, that is not laziness — it is a measurement of how hard your distribution is. Somebody decided a stale copy in their pocket beat the trip to the real one. Fix the trip.

Where a digital permit box fits

Section 05

DryPlans is a distribution tool. It does not do markups, RFIs, or approval workflows, and it will not tell you that Sheet S-2 changed — your architect or engineer does that. What it changes is the geometry of the problem: instead of sending copies of the set, you post one QR placard whose address never changes, and subs and inspectors scan it to read whatever is behind that address right now.

Three parts of it apply directly to revisions.

Per-document visibility
Every document has its own visibility toggle. Flip a superseded set to hidden and it disappears from the public page immediately, without being deleted from your account — so it is out of circulation but still yours if the question of what was current on which date ever comes up.
A pointer, not a copy
The placard, the link, and the QR code stay the same all project. Nobody is holding an attachment that was true in July, because what you handed out was never a file.
Scan counts
You can see how often a job’s code was scanned. A revision week with no scans is a useful warning: the notification went out, and nobody went and looked.
What it does not fix
A sub working from a photo he took in March will keep working from it, and no software reaches into a phone in a truck cab. The habits above are the fix. A tool only decides whether the correct set is the convenient one.

Plenty of teams solve this without any of it — a shared drive with one ruthlessly maintained “Current” folder, a superintendent who reprints and swaps the box the day a revision lands, or a full construction management platform if you are big enough to have someone administering it. All of those work. The common ingredient is not the product; it is that exactly one place is allowed to be right, and everyone knows which.
